Incident Reporting – New Incident

Go to Incident Reporting on the main menu.

When adding a new Incident, click on the ADD NEW REPORT button to complete online.

STEP BY STEP:

  1. Go to Incident Reporting on the main menu.
  2. Click on ADD NEW Button.
  3. Select Print or ADD NEW REPORT.
  4. The incident report form will open, fill in all the details.  The form is conditional, for example: If you select Injury, more fields will open ready to be completed.
  5. Completed all sections.
  6. If you have completed everything click SAVE & SUBMIT.

NOTE:  If you need more information you can Save as Draft and go back and complete.
